Complete a freedom of information request form
The freedom of information (FOI) form is for FOI requests only. Do not use this form to ask questions about health conditions or illness.
If you’re unwell and it’s and emergency, phone 999.
If it’s not urgent, contact your GP. Phone 111 if your GP practice is closed.
What to do if you're unhappy about the response to your FOI
If you’re unhappy with the response to your FOI, you can ask for an internal review of the decision. To do this, you must email NHS24.FOI@nhs24.scot.nhs.uk.
If you’re still unhappy after the internal review, you can appeal. You can do this through the Scottish Information Commissioner.
The Commissioner has more information about your rights under the Freedom of Information (Scotland) Act 2002.
